Adult Registration Fees
Full-sided play – $25/player per calendar year*
- Small-sided play – $18/player per calendar year*
- One weekend (3 day) Tournament only play – $8/player per weekend*
- *Some leagues may charge more to cover administrative costs
Adult Membership Application
Submit the following documentation to Mark Webber at mark@soccerindiana.org and Angel Hall at angel@soccerindiana.org
- Letter requesting membership. Letter must include information about the league: who they serve, approximate number of participants, a description of the league, the geographic area they serve, where they actually play (facilities), complete list of directors and officers with contact information (both phone and email) and job titles.
- Copy of 501c3 declaration letter if they are non-profit
- Copy of Bylaws
- Copy of playing rules.
- Copy Articles of Incorporation/State of Indiana Business Entity Report (current)
Appeals Process
Indiana Soccer utilizes US Youth Soccer’s Grievances, Disputes and Appeals process and complies with US Soccer’s policy(s) addressing the same.
All members of Indiana Soccer, coaches, players, administrators, and parents can appeal a decision, unless the activity, event or competition rules for which the appeal is being sought, limits an appeal to the activity, event or competition administrators or their assigns.
Adjudications that are not appeals limited, must be delivered in writing, to the Indiana Soccer Board of Directors, at the current address of the Indiana Soccer office. The appeal must be sent registered mail; marked to the attention of the Executive Director and be accompanied by an appeals fee of $2,500– [twenty five hundred dollars], not later than 10 days after the date of the decision.
The appeals fee will be returned should the decision for which the appeal is filed be overturned.

- Accident Medical Expense Benefit $5,000 maximum benefit
– Accident Dental Expense Benefit (sound, natural teeth only) $1,000 maximum benefit
– Deductible Amount $400 of all eligible expenses
– Accidental Death Benefit $5,000 principal sum
– Accidental Dismemberment Benefit $5,000 principal sum
– Hospital Room & Board Expenses (In-Patient) $300 maximum per day
– Hospital Miscellaneous (In-Patient) $1,000 maximum per admission
– Hospital Miscellaneous Expense (Out-Patient) $250 per admission
– Hospital Emergency Care $350 maximum per injury
– Physicians Expense (Non-surgical) $35 maximum per visit limit, 10 visits per injury
– Surgeon Expense (In- or Out-Patient) Allowed at 50% of the Usual, Reasonable & Customary (UCR) amount
– Assistant Surgeon Expense Allowed at 25% of surgeon’s UCR
– Anesthesiologist Allowed at 25$ of surgeon’s UCR
– Physical Therapy or Chiropractic Expenses $25 maximum per visit, limit 15 visits per injury
– X-rays (In- or Out-Patient) including diagnostic
– Imaging, MRI, CAT Scans, or similar procedures $150 maximum per injury
– Ambulance Expense $150 maximum per injury
– Orthopedic appliances or braces as a result of covered injury NOT for the prevention of injury $400 maximum per injury.
Accident Medical Coverage upgrades available by league:
